USB Power Cable, Optical Cable, and A40 Inline Mute Cable. Sounds like your settings are causing the issue, The PS4 needs to have USB for voice and then optical for audio, which is like Dolby bitstream etc, And then you need to have the PS4 on chat audio. Curious which mixamp it is, because if it's the pro then nothing needs to be connected to the controller and if you do then it's set up wrong outside of using the usb to power it optionally.

I noticed in the PS4 audio device settings, the volume control (headphones), and output to headphones is grayed out while the output device is set to TV or AV amplifier.

In order for me to have the chat play through the stream, I have to have the output through PS4 set to "tv or av amplifier".

Since I initially didn't know which mixamp it was and the pro tr has firmware/software that does need to be updated. Its best to think of it this way, the "Game" side is the optical connection and the "Voice" side is the USB connection.
